The incident occurred despite the Safe{Wallet} user interface displaying correct transaction details related to Bybit; a malicious transaction carrying a valid signature was executed on-chain. The preliminary findings are as follows:
- Codebase Security: A comprehensive audit revealed no signs of intrusion or unauthorized modification to the Safe codebase.
- Dependency Check: No malicious dependencies affecting the transaction flow were identified, ruling out supply-chain attacks.
- Infrastructure Status: System logs show no evidence of unauthorized access to infrastructure.
- Impact Scope: No other Safe addresses have been confirmed as affected.
To ensure platform security, Safe{Wallet} has implemented the following measures:
- Conducting a full review of all service configurations.
- Rotating and updating all infrastructure access credentials.
